Cerebral tuberculoma (CT), a localized form of tuberculous meningitis, is a disease that is difficult to diagnose because of its nonspecific clinical manifestations \[[@REF1]\]. It can present as multiple, space-occupying, well-circumscribed lesions in the brain. These lesions can be either hypodense, isodense, or hyperdense. CT is commonly found in cases of tuberculous meningitis, central nervous system lymphoma, and leptomeningeal metastases \[[@REF2]\]. To our knowledge, few cases of cerebral CT have been reported as a cause of posterior fossa subdural effusion (PFSE) \[[@REF3]\].

Case presentation

A 40-year-old woman with no comorbidities was transferred to our hospital due to altered mental status. Initial examination revealed signs of meningitis, including decreased level of consciousness, nuchal rigidity, and Kernig’s sign. A computed tomography (CT) scan of the head showed a large and irregular intra-axial mass lesion in the right posterior fossa with subdural extension (Figure [1](#FIG1){ref-type=”fig”}). A non-contrast enhanced magnetic resonance imaging (MRI) scan of the brain (Figure [2](#FIG2){ref-type=”fig”}) revealed an isodense lesion with a cystic component on the sagittal image. The lesion was hypointense on the T2-weighted image, and was mainly hyperintense on the T1-weighted image.
