Myocardial salvage after successful coronary thrombolysis.
The authors sought to determine whether myocardium salvage after successful coronary thrombolysis is dependent on the rate of lysis. In 18 dogs, arterial occlusion (60 minutes in duration) was produced in the anterior descending coronary artery (LAD). Infusion of a mixture of tissue-type plasminogen activator (120 mg/kg) and urokinase (100,000 U/kg) was begun 2 hours after occlusion. Dogs were randomly divided into three groups, and a second infusion of agents (200 mg/kg) was done 2 hours after the first infusion. Eight dogs were killed 2 hours after second infusion, and the remaining 10 were killed 8 hours after second infusion. Thrombi were extracted from the LAD at the time of second infusion, and size of the thrombi was measured. A collagen surface area scan was performed to determine percent salvage. Infusion of plasminogen activator and urokinase increased plasmin activity more than 1,000-fold above baseline and resulted in substantial coronary thrombolysis. Scanning in all dogs showed a mean salvage percentage of 46% +/- 8.5%, with no difference between the two groups. A second lysis and infusion resulted in salvage in 50% of the dogs. If plasminogen activator and urokinase therapy is started within 2 hours of ischemia, it is not the rate of lysis but the volume of blood flow in the ischemic myocardium that determines which areas of myocardium are salvaged and the amount of salvage.Q:
